In-Home Supportive Services

A Medi-Cal program that is funded by county, state and federal dollars; allows low-income elderly, blind and or disabled persons, who are at risk for out-of-home placement, to remain safely at home by providing payment for care provider services.

Medi-Cal and CalFresh

Help to pay medical expenses not covered by MediCare, or assistance with nutrition

Older Adult Mental Health

Care and consultation to individuals 60 years of age and older who suffer from mental illness

Multipurpose Senior Services Program

Provides assistance to frail adults, age 65 and older, to live safely at home and delay or avoid the need for placement in a skilled nursing facility

Public Conservator/Guardian/Administrator

Provides conservatorship services for those who are unable to provide for their own basic food, clothing, and shelter needs, or are unable to manage their personal financial affairs or are susceptible to fraud or undue influence.

Provides individualized intervention to those clients who are medically fragile and most at risk of poor health outcomes
