How to request your records:

If you would like to request your records from the Mental Health Division, Comprehensive Services for Older Adults (Older Adult Mental Health), or the Alcohol and Drug Services Division contact Medical Records at 707-253-6163. Medical Records will set up a convenient time and place to meet with you at Napa County HHSA South Campus, Building A, typically at the reception area where you receive services.

What do I need to bring?

A picture ID is needed to request and pick up records. The form used to request your records will be given to you to complete at the time of your appointment.

How long will it take to get my Records?

It can take up to 15 days to receive your records after a valid request is received.

How much does it cost to get my records?

Napa County HHSA currently does not charge clients for copies of medical records.

Can I come in to review my records?

Yes, by appointment only. Please contact Medical Records at 707-253-6163.

Can you mail my records to me?

Yes, if you provide us with your current mailing address. You may also pick your records up at Napa County HHSA South Campus, Building A.
