Public Health Nursing - 
Public Health Nurses provide individualized intervention to Older Adults who are medically fragile and most at risk of poor health outcomes.

 - Screening and eligibility determination for older adults in need of help covering medical costs not covered by Medicare or other health insurance.

Mental Health - 
Information on case management (treatment and consultation to individuals 60 years of age and older who suffer from mental illness) and Older Adult Full Service Partnership Program (supports chronically mentally ill, under-served, at-risk adults age 60 years and older).

Prescription Drug Discount Card
 - Free prescription drug discount cards that offer a substantial savings off the retail price of commonly prescribed drugs.


Adult Protective Services
 - Protective services to elderly and dependent adults who are at risk for abuse, neglect, or exploitation by themselves or others.

In-Home Support Services - 
In-Home Support Services allows low income older adults who are at risk for out-of-home placement, to remain safely at home by providing payment for care provider services.

Caregiver Permit Program
 - Information on the caregiver ordinance and how to apply for, or verify, a caregiver permit.

Public Conservator-Guardian-Administrator
 - Information on court appointed conservatorship services which help manage someone’s personal needs or financial affairs.

Cash Assistance Program for Immigrants
 (CAPI) - CAPI is designed to provide monthly cash assistance to aged, blind, and disabled non-citizens who are ineligible for Supplemental Security Income/State Supplementary Payment (SSI/SSP) solely based on their immigrant status.
