How to Report a Disease

Refer to page three of the CMR to determine the reporting requirement.

Report Immediately

Immediately report to the Napa County Communicable Disease Unit at Public Health, by filling out the CMR Reporting PDF  PDF and faxing to 707-299-4479 or emailing to [email protected].

• Napa County Answering Service staff will page the on-call Communicable Disease Duty Officer.

Report within One Working Day

• Complete Confidential Morbidity Report (CMR) and email to [email protected] 
• For questions please call: 707-253-4231
• Note: To maintain patient privacy, please password protect the CMR file and email the password in a separate email to the same address.

Report within Seven Calendar Days

• Follow the instructions for reporting within one working day
